I just noticed this in the status of my California EDD DI claim. I had to leave my job based on doctor recommendation last October. After I left the job, there was a temporary lapse in the payments I was receiving from EDD, but they picked back up again late Oct. When they started paying me again, it stated my claim was in automatic payment mode, no action required, and that my expected return to work date was 1/1/2017 (even though I had to quit my job, which they are aware of). Like I said, the status has changed from "no action required" to "To continue to receive payments, submit the Continuing Eligibility Questionnaire (DE 2593)". Problem is, I can't find that form or record of it on the EDD site anywhere, or via Google search. I can only think that it will be mailed to me. Maybe I'm paranoid, but I'm worried that I will stop receiving benefits. I was hoping that they'd continue paying me until this May, when the money runs out, or until I find a job I can do (currently renewing my IT certs). Do they think I'm not eligible all of a sudden? Should I be worried? Is it normal to have to fill out this questionnaire to continue benefits? Lastly, I no longer have a doctor, I had to stop the appts. because I couldn't afford them without benefits. The doctor sent me a letter of dismissal. So if EDD wants more doctors notes, I guess I'm screwed.

Well, EDD sent the questionnaire to my inbox late last night. I filled it out honestly, that I did not return to work, nor am I receiving any other form of income. It asked if I was still under medical care and I had to put no since I stopped treatment for the permanent arthritic condition. I hope they do not stop my benefits because I am no longer attending regular doctor appts...does anyone know how that will play out? Last edited by metalone4; 01-08-2016 at 05:17 PM..

They cut off my disability benefits as soon as I told them I was no longer under the care of a physician. My condition is permanent and not going to improve, my medical benefits were no more, so I stopped scheduling appts. If you want to remain eligible for disability payments you must remain under a physician's care the entire duration. As long as you're eligible in the first place and have your doctor send in updated paperwork, you should be ok. If I had it to do over again, I would have applied for MediCal benefits sooner, had my medical records transferred to a new doctor, and then had them fax in updated paperwork. Sorry to hear that you were cut off; I hope you're able to get continued benefits since you're still disabled. For future reference, you do need to re-certify that you're disabled to continue to receive benefits. (Apparently this is still true if your disability is permanent.) And generally that requires an update from the professional you're seeing for treatment. I think the update is quarterly. I recommend in the future (or now if necessary) that you get an update from whoever was treating you. You may need to pay them for the assessment. If you can't afford the same person, hopefully there are cheaper alternatives. You might also try negotiating a no-cost update with your treatment provider. I need to fill out the quarterly form that you mentioned, and I am currently ticked off that it's not available online. Their entire online experience leaves much to be desired. .
